A note from Dr. Ciszak
I occasionally accept new patients, but on a very limited basis, depending on how busy the office is at the time and on the particular circumstances of the person seeking care.
If you would like to find out if I can accept you as a new patient, please provide some information about yourself on this form and I will personally respond to you by email within 1-2 business days (unless I am away).
My decision will be based only on the following criteria:
- how hectic the office is at the moment
- how badly you need medical care
- your geographic location
- whether I think that I would be able to satisfy your needs
If you would rather not fill out a form and wait for an answer, the other option for you is to contact the College of Physicians and Surgeons and find out which doctors accept new patients without limitations.
